What is a suction catheter?

 Coronary thrombus suction catheter is a cardiac interventional treatment based on percutaneous transluminal coronary angioplasty, using the principle of negative pressure aspiration to suck out the thrombus through a suction catheter.


Indications

(Mostly suitable for high-risk acute coronary syndromes with heavy thrombotic load)

1. Enlarged blood vessels related to infarction.


2. The lesion is not completely occluded, and the lesion contains a lot of thrombi.


3. The occlusion shape is the one with the broken ends cut off.


4. There is a floating thrombus near the occlusion.


5. There are many thrombi in the proximal part of the occlusion, and the length of the thrombus is greater than 5mm.


Complications of thrombus aspiration

1. The drop in blood pressure and heart rate during manual suction can be seen in:

(1) Vascular occlusion or spasm;

(2) Recanalization of blood vessels leads to perfusion injury;

The suction catheter should be withdrawn in time and treated accordingly in either case.

Emboli fall off during thrombus aspiration, and withdrawal can lead to non-infarction-related arterial embolism and cerebral embolism.


2. Mechanical thrombus aspiration, the most common complication is bradyarrhythmia, which a drop in blood pressure may accompany.


Contraindications

1. Patients with stable angina.

2. Those with small vessel disease.

3. Severe vascular calcification lesions are not easy to pass.

4. Chronic complete occlusive disease.

5. Patients with coronary artery dissection.
